Certificate Replacement
To request a duplicate certificate:
1. Send a notarized statement:
A notarized statement confirms your identity and protects the confidentiality of your training records. Note: Only if your request is being mailed from outside the United States, and bears an international postmark, is the notary requirement waived. All requests must bear the signature of the UI trainee who is requesting the duplicate diploma.
2. The statement should include:
the address to which the diploma should be mailed
your full legal name
University of Iowa ID number or last 4 digits of your SSN
Full name of training program
Start and end date of training program
Send a $50 check payable to Graduate Medical Education, University of Iowa.
The fee covers the cost of printing and mailing.

Residents and fellows may request historical verification of malpractice claims by sending a completed Authorization for Release Form to:

Verification of Training
To request verification of training, please send an email or letter of request.
The request letter should include the following information:
Mailing Address or email address for entity requiring verification
Full legal name at time of Iowa training
Current legal name
Full name of Iowa GME training program
Start and end date of Iowa GME training program
Last 4 digits of SSN
NPI Number
Signed Authorization for Release of Information (Release Authorization Form)
Notes Regarding Verifications:
- Licensure board requests from (e.g., FCVS) must be completed by a physician and therefore will be forwarded to the current program director for response.
- The Office of GME will only verify training completed under GME contract that were sponsored by UI Health Care.
- Verification requests may require up to two weeks to process.

- OSCEP coordinates the University of Iowa College of Medicine’s community-based medical education – involving medical students, residents and physician assistant students – through six regional medical education centers affiliated with the College. OSCEP also oversees the UI-Affiliated Family Practice Training Network and the College’s Visiting Professor Program. In combination, these community-based medical education programs constitute the Statewide Medical Education System.
