One fellow is selected every other year for this two-year ASOPRS-approved fellowship. Applicants must have completed an Ophthalmology residency and be eligible for licensure in the state of Iowa.

Please register with the Ophthalmology Fellowship Match and obtain a match number.

• 3 letters of reference (one of which must be from the Chair of your department)
• Personal Statement
• USMLE Scores
• US Citizenship or Immigrant Status
